Referrals
If you or your client/patient would like support from a health trainer please fill out one of our referral forms.
Please email or post your completed referral form to the relevant health trainer for your area, marking the envelope "Private and Confidential".
If you are not sure which health trainer to send to please email or post the completed referral form to tracysmith2@nhs.net who will pass on to the relevant health trainer.
We are currently unable to take referrals for the following areas:-
Brislington
Stockwood
Whitchurch
Our Health Trainer Lead, Mandy Shute, would be happy to come and talk to your teams or community groups about health trainers.
